About You
Who is this application for?

This helps us know where to focus our outreach so more families find us.

Diagnosis & Centers

If you are not sure of the exact subtype, that is OK. Write what you know.

Current treatment center

Where the patient is currently being seen. Enter “None” if not yet receiving treatment.

Requested specialist center

The center you are requesting to travel to.

The more context you give, the better the board can advocate for you.

Approximate is fine. We confirm exact dates with you later.

Trip type
Care Team
Do you have a hospital social worker assigned?

Many cancer centers assign a social worker or patient navigator to help coordinate care. Knowing who they are helps us coordinate logistics with them.

Your Request
What can we help with? (select all that apply)

Select at least one. Checking multiple does not mean we cover all of them; the board reviews each request based on your situation and current funding.

Will a companion travel with the patient?

We can cover travel for the patient plus one companion (spouse, parent, caregiver).

The more specific you are, the more accurately we can match what you need.

Your Situation

No tax returns. No income proof. Tell us in your own words and we will trust you.

This helps the board understand your situation. One short paragraph is enough.

Supporting Documents

A physician letter, pathology report, appointment confirmation, or referral. Uploading now speeds review; we can request later if you do not have it handy. PDF, JPG, or PNG. 5MB max.

Common questions

Who qualifies for a travel grant from Beat the Odds Sarcoma Fund?

Sarcoma patients, or someone applying on their behalf, whose specialist sarcoma center is around 100 miles or more from home, with an appointment scheduled or planned within the next 3 months. Close on distance or unsure? Contact us anyway. We look at every situation.

What can the grant cover?

Flights, a hotel near the treatment center, ground transportation (rideshare, taxi, rental car, or gas reimbursement), and train or bus fare, for the patient plus one companion. We pay vendors directly. The board reviews each request based on your situation and current funding.

Do I need to provide tax returns or proof of income?

No. We ask you to confirm in your own words that paying for this travel yourself would be a real hardship. No tax returns and no income documentation are required.

How long does a decision take?

A person reads every application, usually within about 5 business days. Urgent cases are expedited.

Can a family member or caregiver travel with the patient?

Yes. Grants can cover travel for the patient plus one companion, such as a spouse, parent, or caregiver.
